Insurance Providers

While Daniels Family Dentistry is no longer a preferred provider for United Healthcare we will continue to accept all United Healthcare Insurances. Our staff will still submit all claims on your behalf. If there is a difference between our fees and what United Healthcare will cover, that difference will be your copay. Our office has only been in-network with United Healthcare since January 1st, 2020 and will continue to accept UHC as we have prior to that.

We would ask that for policy specific questions you call your company’s insurance representative to find out what your policy will cover out of network. If you have any additional concerns please call us at (309) 944-6125.

Here at Daniels Family Dentistry our mission is to provide you with high quality, affordable treatment that fits your budget. We offer many payment options to meet your needs. Our office accepts payments by cash, checks, Visa, MasterCard, American Express and Discover credit cards. If you have any questions about payment, ask us! We will help guide you through your options and work hard to accommodate your needs.

We accept most insurance plans and are a preferred provider for Delta Dental. If your insurance covers dental treatment, you will receive the benefit of reduced out of pocket costs. Insurance policies vary. We will review your insurance to determine the best method of payment. Once treatment has started, we will file your claims and help you understand your policy to maximize benefits to you.

Please remember that insurance is a benefit for you or your child. If during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
